The SOUND Mobile Rapid Response Crisis Teams are specially equipped two-person units, traveling in vans, staffed by behavioral health professionals and certified peer counselors. They are designed to de-escalate mental health and substance use crises, provide immediate support, and connect individuals to long-term care.

PATH is here to support adults and families who are experiencing homelessness and facing challenges such as serious mental-health concerns, substance use, or physical health barriers. We understand that navigating services can be difficult, and we are here to help you access care and resources while supporting your wellbeing.
Approach to Care
Our PATH team works with you to understand your unique needs and barriers to housing. We create a personalized plan to help you access benefits and essential services such as food, shelter, hygiene, healthcare, mental-health support, and substance-use treatment. We can also connect you to transportation, vocational training, and other resources that help you live with stability and independence. For those at risk of losing housing, we coordinate with emergency service providers to support your immediate needs.
Our team can:
• Meet you where you are, including on the streets, in shelters, in encampments, or through referrals from hospitals, social service agencies, and community members.
• Support access to meals, community programs, and public or private services.
• Connect you to healthcare, mental-health and substance-use resources.
• Assist with housing applications and benefits.
